Full-Time Care Management Specialist - Mental Health

Reports To: Manager of Mental Health


Lower Manhattan/Flushing Queens
New York
United States

Responsibilities:

  • Coordinate mental health treatment for pediatric and/or adult patients and their families
  • Assist mental health clinicians in providing treatment and crisis intervention
  • Provide on-going support and/or case management to patients
  • Pair with providers to carry out depressive collaborative care management
  • Conduct screening of depression and other mental health problems
  • Provide immediate screening for incoming referrals
  • Request and follow up on psychiatric medication pre-authorization
  • Follow up on no-show, at risk patients, ER referrals, and hospitalization
  • Manage Mental Health Direct Line
  • Support mental health providers in designing and conducting workshops and therapy groups
  • Manage care management related patient databases
  • Prepare related reports and documentations
  • Provide community agencies and schools liaison
  • Monitor and manage mental health patient portal
  • Document all service contacts through electronic medical record
  • Attend related treatment meetings
  • Perform other duties as assigned

Minimum Qualifications:

  • Bachelor's degree in social services, psychology, or other related fields
  • Capacity for working in weekends and early evening hours required
  • Strong interpersonal and communication skills
  • Ability to work independently
  • Bilingual in English and Chinese (Mandarin and/or Cantonese) required
  • Strong knowledge of computer software

Compensation and Benefits:

  • Salary: $40,000.00 - 45,000.00 per year
