Full-Time Care Management Specialist - Mental Health

Responsibilities:Coordinate mental health treatment for pediatric and/or adult patients and their familiesAssist mental health clinicians in providing treatment and crisis interventionProvide on-going support and/or case management to patientsPair with providers to carry out depressive collaborative care managementConduct screening of depression and other mental health problemsProvide immediate screening for incoming referralsRequest and follow up on psychiatric medication pre-authorizationFollow up on no-show, at risk patients, ER referrals, and hospitalizationManage Mental Health Direct LineSupport mental health providers in designing and conducting workshops and therapy groupsManage care management related patient databasesPrepare related reports and documentationsProvide community agencies and schools liaisonMonitor and manage mental health patient portalDocument all service contacts through electronic medical recordAttend related treatment meetingsPerform other duties as assigned
Minimum Qualifications:Bachelor's degree in social services, psychology, or other related fields Capacity for working in weekends and early evening hours requiredStrong interpersonal and communication skillsAbility to work independentlyBilingual in English and Chinese (Mandarin and/or Cantonese) requiredStrong knowledge of computer software
