Wang Community Health Center, an award-winning federally qualified community health center is seeking a part-time Pediatrics Nurse Practitioner to join our practice. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers.We do not require hospital rounding. Call is limited to telephone only. We also provide opportunities for teaching, research, and community outreach.Responsibilities:Involve with direct patient physical assessment, education, and health planningMaintain continuity of care by regular patient follow-upAssist in the coordination of services with other disciplines and patient's insurance plansAdvocate for patient in arrangement of medically necessary servicesCommunicate with community's resources to ascertain adequate assistance for patients in needRefer patients to specialty services when necessaryConsult with collaborating physician and other members of the health care team to establish plan of careParticipate in quality assurance process to assure optimal care deliveryProvide comprehensive case management services that include emphasis on health promotion and disease preventionMonitor patient outcome in accordance to the practice protocolsCollaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planningAct as a resource person for the nursing staffAble to work 3 days per week including weekendsPerform other duties as assigned

Responsibilities:
Minimum Qualifications: Graduated from approved Nurse Practitioner ProgramLicensed as Nurse Practitioner in New York StateBasic CPR (BCLS) certification requiredAt least 3 years of clinical experience as a Pediatrics Nurse PractitionerSpecialty Certification preferredBilingual in English/Mandarin-Chinese/English or English/Spanish required
