Full-Time Registered Dietitian - Manhattan

Reports to: Chief of Internal Medicine, Manhattan

Responsibilities:

  • Assist primary care providers to manage patients with nutrition related conditions
  • Assess nutritional needs and food patterns of patients, families, and the community
  • Provide nutritional counseling and education to children and adults to improve or maintain their health
  • Plan and provide individual and group education instruction based on patient needs assessments
  • Maintain patient records, and program records, reports, and statistics
  • Prepare nutrition educational materials and educate the community on nutrition topics in various formats in conjunction with the Health Education department
  • Develop and maintain nutrition quality assurance program and participate in Quality Assurance, Peer Review, and other quality improvement activities to improve the safety and quality of patient care and care delivery
  • Assist in supervision of other staff delivering nutrition education and counseling
  • Assist to teach interdisciplinary staff on nutrition topics
  • Assist in care management of chronic diseases and high risk patients
  • Maintain professional qualifications through continuing professional education and other professional activities
  • Work with the interdisciplinary care team to improve individual patient’s health and health center care processes
  • Able to work independently
  • Perform other duties as assigned

Minimum Qualifications:

  • Current New York State Certified Dietitian-Nutritionists (CDN) license and registration required
  • Registered Dietitian (RD) from the Commission on Dietetic Registration (CDR) required
  • Basic CPR (BCLS) Certification required (Health Center will provide if needed)
  • Certified in Diabetes Care and Education Specialist (CDCES) preferred
  • Previous clinical diabetes management experience – preferably in community setting
  • Bilingual in English/Chinese required

Compensation and Benefits:

  • Salary: $65,000.00 - 85,000.00 per year
