Full-Time Primary Care Physician

The Primary Care Physician is responsible for providing excellent evidence-based primary care with dedicated multi-disciplinary team support in an urban outpatient practice. The incumbent will provide holistic care, be interested in his/her patient’s well-being, coordinate care with specialists, learn from and teach other staff at different educational levels. There will be rotating schedule for after-hours phone coverage, weekends required at weekend pay rate and no in-patient responsibility.Responsibilities:Responsible for the medical care of a panel of adult patientsWork closely with the care team to optimize the health of patients, provide preventive care, diagnose and treat chronic and acute illnessesParticipate in all activities necessary to implement the multi-disciplinary team approach for the care of adult patientsParticipate in Quality Improvement and Quality Assurance activities of the Health Center and the Internal Medicine service to improve the health care of patients, including peer review, medical and regulatory education, and risk management trainingSupervise and teach nurse practitioners, nursing staff and medical students as neededMeet minimum productivity targetsMaintain medical privileges and credentialingObtain and maintain privileges at a local hospitalPerform other duties as assigned

Minimum Qualifications:
Graduate of an accredited medical school and licensed to practice in New York StateCertification or eligible for certification by the American Board of Internal Medicine, American Board of Family Medicine, American Osteopathic Board of Internal Medicine, or American Osteopathic Board of Family PhysiciansKnowledge of a second language (such as Mandarin, Cantonese, Bengali, Korean, Spanish or other languages) preferred
